New Client Info
Welcome to Poland Animal Hospital! Thank you for considering us to care for your pet!
We are excited to be accepting new patients! Please note that we are booking 1 month out for new and inactive clients. We do require a non-refundable deposit of $95.00 for the first visit. This deposit applies directly to the cost of the appointment for the animal and is not an additional charge that you will incur.
If you would like to establish yourself as a client with our hospital, please complete our New Account Request Form and forward your previous medical history to polandah@yourvet.com.
As soon as your account is created, we will send you a welcome email and ask you to call at your earliest convenience to schedule your pet's appointment.
We can process a credit card over the phone for your deposit and email you the receipt.
If you do not currently have a vet and your pet needs medical care, please reach out to your nearest animal emergency hospital or urgent care.
